Health Forms Instructions:
● All students, not just athletes, must complete the physical form packet in the 2026-2027 Health Packet PDF below.
● Physicals must be completed on Morris Catholic forms, no exceptions.
● Physicals are good for 1 calendar year.
● Medication and Supplemental forms are good for 1 school year only.
- All students must submit a current physical (2026 - 2027) that is signed, stamped, and dated by your child’s doctor, and a current immunization record to be cleared to start at MCHS.
- If your child has severe allergies, asthma, diabetes, or seizures, a medical action plan will need to be completed and signed by their doctor.
- Per NJ law, all medications taken in school, including over-the-counter medications, require a Medical Authorization Form to be completed and signed by a doctor.
- Each of these forms must be completed every school year. All physicals, forms, and immunization records should be mailed or dropped off at the school, addressed to the Health Office.
- The Interscholastic Athletic Form must be completed and turned in at the time of enrollment.
No student is allowed to attend school, participate in physical education, theater, or athletics without a valid physical. Please be sure to keep a copy of all forms for your records.
